Lodging
Your hotel room and tax for Thursday night will be Master Billed. If you’d like to extend your stay at your own expense, please email events@caredx.com for room rates and availability.

Transportation
For transportation to and from event, please utilize taxi or any app-based car service.

Reimbursable Expenses
CareDx will reimburse reasonable travel expenses costs including transportation, lodging, and related expenses for attendance at this event. All travel arrangements must be made through CareDx's travel agent at CTI Business Travel.

In order to quickly and accurate reimburse your travel expenses, please complete the linked Vendor Financial Data Form and W9 and send it to vendorsetup@caredx.com and events@caredx.com

Please save all itemized receipts and email to events@caredx.com upon completion of your program. (Please note, all reasonable travel expense reimbursement is subject to review and must be in alignment with our travel policy, including daily meal limits, no resorts/spas.)

Payment Processes
Additionally, you will be asked to sign a contract for your attendance at this workshop. Please expect to receive this in the coming weeks.

Once you provide your information to set-up direct deposit, you will be called by CareDx to verify your banking information. It is critical that you answer this phone call and verify your information before we can release payments.
